To get on the Waitlist

• Ask your medical doctor fax us a referral.

  • We accept referrals from: Primary care doctors, Pediatricians, and Psychiatrists (MDs).

  • We cannot accept referrals from: therapists, counselors, or psychologists.

Referrals

• We are accepting referrals for people ages 2 through 30.

  • We are concentrating our efforts on the needs of the younger members of our community.

  • If you are age 31 or older, please ask your medical provider for a referral elsewhere.

• This change does not apply to you if:

  • You already completed a phone intake with our Practice Manager, Julie.

  • We received your doctor’s referral prior to 10/04/22.

We do not offer assessments for: SSDI (disability) benefits or legal proceedings, including custody cases.

Disclaimer:

Our providers have an extensive Waitlist for evaluation services.

• Being on our Waistlist means that:

  • You were referred to our office, or have asked to be added to our list of people requesting an evaluation.

  • You are "in line," and will be contacted for a phone intake after we receive a medical referral.

    • Our psychologists will review your information to determine if they can accept the referral.

• Being on our Waitlist does NOT mean that:

  • You are guaranteed to become a patient in our practice, or that you will be evaluated by one of our psychologists.

  • You are an established client of our one of our providers, or that one of our providers will act as your psychologist in any manner until your first scheduled appointment is complete.
