The Evaluation Process

Step 1: Intake by Phone

  • Julie, our Practice Manager, will call to get info from you, such as:

    • Your insurance info

    • Referral information

    • Relevant names and phone numbers

Step 2: Paperwork

  • Watch for an email from Julie. This email will give you instructions on:

  • Gathering necessary records. Examples:

    • School transcripts and a copy of 504 Plan or IEP

    • Parenting Plans or divorce documents.

  • Completing the Intake paperwork.

If needed, you can PRINT a Release of Information (ROI)

Step 3: Schedule the evaluation

Julie will call you to schedule three appointments:

  1. The Diagnostic Interview

  2. The Testing

  3. The Feedback/ Results appointment

Step 4: Diagnostic Interview

  • When: As scheduled, for 1 hour

  • Where:

    • With Dr. Hogansen: TherapyPortal.com

    • With Dr. Scheidel: Use the Zoom link that was emailed to you.

  • What happens: The doctor will interview you to:

    • Understand your concerns and goals.

    • Determine if she can meet your needs.

After this appointment, you and the doctor can decide if you want to proceed with the evaluation.

Step 5: Testing

  • Arrive at the office, as scheduled.

  • Your doctor will:

    • Come out to the waiting room to get you.

    • Answer any questions, and make sure you are (your child is) comfortable.

    • Your provider may ask you to complete additional paperwork.

  • The doctor will administer relevant, standardized tests.

    • There will be short breaks throughout the test session.

    • Full-day test sessions include a 30-60 minute lunch break.

    • If your child is being assessed, you or may leave the building if:

      • Your child is 8 or older.

      • They are comfortable with you leaving, and you will be available via mobile phone

      • You will pick them up for the lunch break (if applicable).

Step 6: Feedback/ Results appointment

  • When: As scheduled, for 1 hour

  • Where:

    • Dr. Hogansen: TherapyPortal.com

    • Dr. Scheidel: Use the Zoom link that was emailed to you

  • Who:

    • Child client: Parents only.

      • If your child is 16 or older, you can decide if they should participate.

    • Adult client: You. Your partner can attend, if you wish.

  • What happens:

    • The doctor will:

      • Walk you through her findings.

      • Provide a tailored set of recommendations

      • Answer your questions.

Step 7: Get your report

  • When: Your comprehensive report will likely be done 4 - 8 weeks after the Feedback/ Results appointment.

  • How: You will download it from the Patient Portal.

  • What the report includes:

    • The doctor’s findings and diagnoses (if any)

    • Specific recommendations

  • Who else gets the report:

    • We will fax copies to whoever you ask us to (ex: school, therapist) — if we have a Release of Info (ROI)

    • You can give copies of to whoever you want— it’s your report
